Confidentiality

Client Confidentiality Agreements

When a new person is hired or retained or volunteers or is an intern, the individual will sign a confidentiality agreement on their first day of work.  Each staff member will sign the appropriate forms annually thereafter.  The original agreements will be kept on file at the State Office.  (See Appendix: Forms 300, 301, 302, and 303 and see also Professional Conduct, Code of.)  

 

The forms that need to be signed are:

 

Support Staff Professional Staff:

 

1. Conflict of Interest Standards of Conduct Staff Agreement (Form 300)

 

Professional Staff:

 

1. Code of Professional Conduct (Form 301)

 

2. Conflict of Interest Policy for Professional Staff (Form 302) or

 

3. Conflict of Interest Policy Addendum for Professional Contracted, Volunteer & Intern Staff (Form 303)
